The enrollment contract and the financial obligation statement must be signed and returned to the School office with the required, non-refundable registration fee. When accepted by the school, a copy of these agreements will be returned to you.

I request to enroll the student named above in St. Matthew’s Episcopal School for the 2010-2011 school year and agree to pay $4,130 annual tuition (Pre-Kindergarten – 7th grade).
It is agreed that:
The registration fee, required to be returned with this enrollment contract to hold the above mentioned student’s place, is non-refundable.
I understand that once I enroll my child in St. Matthew’s Episcopal School (after acceptance), I have the obligation to pay the entire tuition by May 14, 2010.

I understand that if I withdraw my child from St. Matthew’s Episcopal School for any reason after registration, the tuition will be refunded as follows:
Prior to the first day of school – 75% of annual tuition
Prior to September 30, 2010 – 50% of annual tuition
After September 30, 2010 – no refunds
I understand that if at any time, in the opinion of the School, enrollment of the student shall be deemed detrimental to the student’s health or progress or to other students, he or she may be dismissed. The student is also subject to disciplinary action, including suspension or dismissal, for violation of school rules.
I agree to provide all of my child’s academic, medical and psychological records and evaluations to SMES.
Unless written notice to the contrary is received in advance and acknowledged in writing by the administration, the student shall be deemed to have permission to take part in any and all school activities on or off campus and to ride in vehicles driven by St. Matthew’s Episcopal School faculty and staff members or parents or in vans chartered by St. Matthew’s Episcopal School. I hereby release St. Matthew’s Episcopal School, its trustees, faculty, staff, parents and students from liability for any injury to the person or property of the students by reason of his or her participation in such activities or riding in such vehicles.
I understand that in signing this enrollment contract for the 2010-2011 school year, I am agreeing to accept the rules, regulations, policies and procedures of St. Matthew's Episcopal School, including rules concerning payment of fees as referred to above.
